PERSISTENT SHOULDER PAIN: A COMPREHENSIVE APPROACH FROM DIAGNOSIS TO SPECIALIZED TREATMENT

On the morning of August 9, 2025, Hanoi French Hospital hosted the workshop “Persistent shoulder pain: Comprehensive care from medical treatment to surgery and rehabilitation,” attracting a large number of attendees.

Opening the program, Dr. Dang Ngoc Tan – Department of Rheumatology, Hanoi French Hospital – presented an overview of the groups of causes leading to shoulder pain. According to Dr. Tan, shoulder pain is not only the result of injury or degeneration, but can also be related to arthritis, systemic connective tissue diseases, or central and peripheral nervous system disorders.

The incidence of shoulder pain increases with age and is more common in women, especially those whose occupations require repetitive movement, heavy lifting, or maintaining uncomfortable positions for long periods. In addition, lifestyle factors such as smoking, alcohol consumption, vitamin D deficiency, comorbidities, or stress, anxiety, and insomnia also contribute to worsening the condition,” Dr. Tan shared.

In terms of treatment, based on the guidelines of the European Alliance of Associations for Rheumatology (EULAR), the doctor emphasized the principle of individualized treatment, combining non-drug and drug therapies, as well as considerations for intra-articular injections. In particular, increasing patient awareness has a significant impact on pain control effectiveness.

At the event, Dr. François Duroux – French Orthopedic and Trauma Surgery specialist at Hanoi French Hospital – provided detailed information on the indications and procedures for shoulder surgery, with a focus on arthroscopic shoulder surgery. This technique can treat up to 90% of shoulder conditions requiring surgery, especially shoulder impingement syndrome, rotator cuff tears (supraspinatus tendon), recurrent dislocations, and severe arthritis.

“Arthroscopic shoulder surgery is a modern, minimally invasive technique that offers high treatment effectiveness. With an incision of only 3–5 mm and a very low risk of infection, this method helps reduce pain and shortens recovery time,” Dr. Duroux explained.

In addition, Dr. Duroux analyzed in detail commonly applied techniques such as acromioplasty, ligament repair (Bankart), bone grafting (Latarjet), or joint replacement.

For chronic shoulder pain, whether treated medically or surgically, rehabilitation is an indispensable step to restore range of motion and prevent recurrence.

Dr. Nguyen Thi Dung – Head of the Department of Physiotherapy and Rehabilitation, Hanoi French Hospital – said: “Shoulder pain ranks third among musculoskeletal conditions requiring rehabilitation. For the best outcomes in treating persistent shoulder pain, patients should seek medical consultation as soon as the first symptoms appear and strictly follow the prescribed treatment plan. Shoulder rehabilitation not only includes individualized, specialized exercises but also incorporates modern therapeutic methods to effectively control pain without causing long-term complications.”

In addition, patients should pay attention to risk control by maintaining correct posture in daily activities and at work.

At the end of the program, many practical questions from patients were sent to the doctors, covering issues such as night shoulder pain, indications for joint injections in degenerative conditions, surgical indications, and home exercise methods.
