*Note:

• If a customer does not have a vaccine package, they will be requested to pay any additionals costs incurred
• Vaccine excluded in the Package: Hepatitis A
• Regarding Rotavirus: Rotarix must be used before 24 weeks / Rotateq must be used before 32 weeks
• Hexaxim/ Infarix should be vaccinated before 24 months
