PERSONALIZED MANAGEMENT IN HYPERTENSION

Personalized treatment refers to the adjustment of the overall management strategy based on the characteristics of each individual patient. Current medical practice trends apply personalization not only to hypertension but also to chronic diseases in general.

PERSONALIZED MANAGEMENT IN HYPERTENSION

Dr. Pham Thu Thuy, Clinical and Interventional Cardiology Department, Hanoi French Hospital, stated:

Personalized management includes achieving target blood pressure levels while also addressing multiple factors such as comorbidities (other medical conditions), existing complications, individual responses to antihypertensive medications, and each patient’s ability to adhere to treatment.”

Personalization based on overall cardiovascular risk

Two patients may have the same level of hypertension but very different risks of complications. Therefore, patients need to be comprehensively assessed based on risk factors such as age, sex, cardiovascular history, diabetes, dyslipidemia, chronic kidney disease, smoking, and physical inactivity. This forms the basis for determining the intensity of intervention and treatment targets.

Importantly, cardiovascular risk is not static but changes over time. Patients therefore require periodic reassessment to adjust treatment goals and strategies accordingly.

Personalization of blood pressure targets

According to current recommendations, most patients should aim for a blood pressure target below 130/80 mmHg if well tolerated, while the previous threshold of 140/90 mmHg is generally reserved for very elderly or frail patients.

Personalization of treatment regimens

The choice of antihypertensive medications and their combinations should be tailored to each patient’s characteristics. During follow-up, physicians assess treatment response to adjust dosages, optimize efficacy, and minimize adverse effects.

For patients with comorbidities such as coronary artery disease, diabetes, or kidney disease, treatment regimens must both control blood pressure and protect target organs while managing associated risk factors.

Personalization over time

Treatment strategies should evolve according to disease stage:

  • Initial stage: achieve safe blood pressure levels and control associated risk factors
  • Stable stage: maintain treatment, limit excessive blood pressure variability, and screen for early complications. Follow-up should be maintained at least every 6 months
  • Complicated stage: prioritize target organ protection and prevention of recurrent cardiovascular events

Treatment response may also change over time due to factors such as weight, aging, renal function, or the development of new conditions.

Personalization based on context and adherence

An effective treatment strategy must be feasible in real life, allowing patients to sustain long-term management. Patients living far from healthcare facilities may require more flexible follow-up plans. Access to medications and the availability of specific drug classes in local settings should also be considered.

In addition to blood pressure monitoring, physicians assess lifestyle factors, treatment adherence, and the patient’s ability to maintain necessary long-term changes.

At Hanoi French Hospital, personalized management is implemented from the initial stage and continuously adjusted throughout follow-up. Patients are managed by the same physician over time, allowing early detection of subtle changes, timely adjustment of treatment, and optimization of healthcare costs.

In addition, Hanoi French Hospital offers a structured hypertension management program with comprehensive follow-up components, supporting patients in maintaining effective long-term control.
